Diving Medical Statement
Before taking part in a dive, Try Dive or diving course, you’ll need to complete a medical questionnaire. This helps determine whether there are any medical considerations that need to be addressed before you go diving.
In most cases, completing the medical questionnaire is all that is required. However, if any of your answers indicate that further medical assessment is necessary, you’ll need to provide appropriate medical clearance before diving.The document available for download on this page is provided so that you can review the questions before you arrive.
If you’re unsure about any of the questions or think that one of them may apply to you, we recommend discussing it beforehand with a doctor – ideally one with experience in diving medicine.The final medical declaration must be completed and signed at our dive centre before the start of your diving activity.
